Information About Our Products
Do to the nature of our products, we try to provide the most accurate representations of your tests as possible. However, results can depend on your collection, retrieval, and other factors outside New Day Diagnostics’ control and will from time to time have variation in results. The subsequent uniqueness of the result in our product is an acceptable attribute of science and biotech testing. These slight differences are not deemed as material differences or incorrect results.
Consent to Testing and Reporting
BY CONSENTING TO THIS TEST YOU AGREE TO THE FOLLOWING:
- YOU AUTHORIZE THIS TESTING UNIT TO CONDUCT COLLECTION AND TESTING FOR THE ORDERED TEST (i.e. COVID-19, etc)
- YOU INDEMNIFY THE ORDERING PHYSICIAN AND THE TESTING LABORATORY
- YOU AUTHORIZE YOUR TEST RESULTS TO BE DISCLOSED TO THE COUNTY, STATE, OR TO ANY OTHER GOVERNMENTAL ENTITY AS MAY BE REQUIRED BY LAW
- YOU AGREE TO THE REPORTING OF RESULTS TO NEW DAY DIAGNOSTICS AND TO YOUR EMAIL ADDRESS
- YOU UNDERSTAND THAT THE COLLECTING AND TESTING LABORATORY IS NOT ACTING AS YOUR MEDICAL PROVIDER
- YOU AGREE TO FOLLOW UP WITH YOUR OWN PROVIDER WITH YOUR RESULTS
- YOU UNDERSTAND THAT, AS WITH ANY MEDICAL TEST, THERE IS THE POTENTIAL FOR FALSE POSITIVE OR FALSE NEGATIVE TEST RESULTS TO OCCUR
Returns and Exchanges
Our goal is for every customer to be pleased with our product and services. If you must cancel your order, see “Order Cancellation Policy” for the return of unopened, unused test kits and applicable restocking fee. Due to the nature of this business, there are no refunds or exchanges for opened, used, or contaminated test kits and all sales are final. However, in the event that you are not completely satisfied with your purchase, you may contact New Day Diagnostics within 10 days of shipment to see if we can assist in some manner. In the instance a test kit is damaged upon arrival, EDP must be contacted within 48 hours so further steps can be followed to ship a new test and follow up with the shipper.
All returns of a damaged kit must be sent back to New Day Diagnostics within 48 hours. New Day Diagnostics will send an email to you with all pertinent information and must be accompanied with a Return Authorization, which may be obtained by emailing or calling us with the details of your order number, product purchased, picture of the outside of the kit received showing damage. You may receive your Return Authorization by contacting firstname.lastname@example.org. Return Authorizations will expire after 15 days or no replacement kit will be given. Only upon receipt of the original, damaged kit at New Day Diagnostics, will a new kit be shipped to you.
New Day Diagnostics cannot be held liable for damage or negligence or results interpreted by anything other than the intended use found within each kit.
New Day Diagnostics is not responsible for determining testing or reporting requirements for international or domestic travel.
Order Cancellation Policy
Please call us at (865) 299-6250 immediately or email us at email@example.com if you need to cancel an order.
RUSH 24-Hour PCR Test at New Day or Weekend Testing is NON_REFUNDABLE once the order has been processed.
All other orders including, but not limited to, On Location COVID Antibody Test, COVID-19 Diagnostic PCR (nasal pharyngeal swab) at EDP, COVID-19 Diagnostic COVID SalivaDirect™ Test at EDP are subject to a 25% restocking fee if cancelled prior to administration of the test. Testing services are non-refundable once the test has been administered.
Shipped orders that are refused are subject to a cancelation and restocking fee of 25%, not including original shipping charges and any charges for return shipping.
If you have questions about international or domestic travel testing, we encourage you to contact your airline or destination first to determine testing requirements. New Day Diagnostics can help you select the appropriate test for your travel, however, it is your responsibility to ensure that you have purchased the correct test for your travel situation. New Day Diagnostics is not responsible for determining testing or reporting requirements for international or domestic travel.
If your airline or destination requires extra details or written information on your Test Result (i.e., passport number, physician stamp, etc.) you must indicate this need before or during your testing appointment. New Day Diagnostics will make every effort to meet these requirements in advance of your travel. New Day Diagnostics is not responsible for travel delays or flight cancellations and will not issue refunds due to incorrect test purchase or results formatting which were not communicated in advance.
Shipping & Payment Terms
All shipping errors must be reported within 48 hours of receipt of the package. All items listed on invoice should be in the box. In the event that an item is on reserve or backordered, you will not be charged until it is confirmed that you still wish to receive the reserved/backordered item, and then you will be notified when it is available for shipment and billing.
If you believe that you have been short shipped an item, please double-check all packing materials. Please notify us at once for further instructions. DO NOT discard original packaging material or the original shipping box, it will be required it you are instructed to repack and return the entire order back to us.
We track each order individually. We will compare weight, packing materials and shipping notes to determine if there was a short ship. If we determine that there was a short ship, we will only replace the exact item claimed.
Items Lost or Damaged Shipment
If damage does occur in transit it is usually the responsibility of the transportation company. If your item was shipped via UPS or FedEx and arrived damaged, please call us immediately to file a claim with UPS or FedEx. You must save all packing materials so the representative can inspect the package and make a determination of responsibility. If you feel the item damage was not caused during shipping, you must return it to us for inspection first. Please contact us at firstname.lastname@example.org within 48 hours of receipt. Items must be shipped back within 48 hours from time Return Authorization is issued. We will make a determination within 5 days of receipt back in our offices. Claims will be processed within 10 days.
Jurisdiction and Venue
This agreement shall be construed according to the laws of the State of Tennessee, without regard to any conflict of law provisions. Any disputes arising under this Agreement shall be resolved according to the laws of, and in the state and federal courts of, the state of Tennessee, USA. Venue shall be proper only in the County of Knox, State of Tennessee.
Prices are subject to change without notice. We are not responsible for typographical errors or inaccuracies. We reserve the right to correct any errors or omissions at any time without prior notice. We reserve the right to modify or correct information at any time without prior notice. We reserve the right to correct any errors (including pricing), inaccuracies or omissions on any orders, including orders which may have been submitted.
Tennessee law requires us to charge 9.25% sales tax to the cost of the order for all orders shipped within the state of Tennessee. There is no sales tax on orders shipped out of the state of Tennessee.
